These Website Terms of Use govern your access to and use of skanwellness.com, including its content, forms, embedded scheduling features, links, and related online services.
By accessing or using this website, you agree to these Terms. If you do not agree, do not use the website.
1. About SkanWell
“SkanWell,” “we,” “us,” and “our” may refer to:
SkanWell Management LLC, which may operate or support the website and provide scheduling, technology, marketing, management, and other non-clinical services; and
SkanWell Medical Health PLLC, which provides professional medical services through appropriately licensed healthcare professionals.
SkanWell Management LLC does not practice medicine, diagnose conditions, prescribe medication, determine treatment eligibility, or control the independent professional judgment of licensed healthcare providers.
2. Informational and Educational Purposes
Website content is provided for general informational and educational purposes.
The website is not intended to:
Provide individualized medical advice;
Diagnose a health condition;
Recommend a specific treatment for a particular person;
Replace an examination or consultation with a qualified healthcare professional;
Establish a standard of care; or
Serve as a substitute for emergency, urgent, primary, or specialty medical care.
You should consult an appropriately licensed healthcare professional regarding medical symptoms, diagnoses, treatment decisions, medication changes, and other healthcare matters.
Do not disregard professional medical advice or delay obtaining care because of information appearing on this website.
3. No Emergency or Urgent Medical Services
The website, contact forms, online scheduler, email inboxes, and social-media accounts are not monitored for medical emergencies.
Do not use them to:
Report a serious reaction;
Request emergency assistance;
Communicate time-sensitive medical information; or
Obtain urgent medical advice.
If you believe you are experiencing a medical emergency, call 911 or go to the nearest emergency department.
4. No Patient-Provider Relationship Through Website Use
Viewing the website, completing a contact form, requesting an appointment, subscribing to communications, or communicating with SkanWell does not by itself establish a patient-provider relationship.
A patient-provider relationship is established only when:
SkanWell Medical Health PLLC accepts you as a patient;
An appropriately licensed healthcare professional performs the evaluation required for your care; and
Applicable consent, documentation, and clinical requirements have been completed.
SkanWell may decline to establish or continue a patient-provider relationship when legally and professionally appropriate.
5. Treatment Eligibility
Information describing a service, medication, product, device, or procedure does not mean that it is appropriate or available for every person.
All medical services are subject to an individualized clinical evaluation.
A provider may require:
Medical-history information;
An in-person or telehealth evaluation;
Physical examination;
Laboratory testing;
Medical records;
Identification;
Medical clearance;
Follow-up monitoring; or
Referral to another provider.
Scheduling a consultation does not guarantee that any particular treatment, prescription, medication, product, or procedure will be recommended or provided.
6. PatientNow Scheduling
SkanWell uses an embedded PatientNow scheduling feature.
When you use the PatientNow scheduler:
You may be asked to provide personal, health, payment, and scheduling information;
Additional terms, notices, acknowledgments, and consents may apply;
Information may be transmitted directly to PatientNow;
Information may become part of your prospective-patient or medical record; and
PatientNow’s terms and privacy practices may apply to its independent services.
You are responsible for providing accurate, current, and complete booking information.
An appointment is not guaranteed until you receive confirmation.
SkanWell may contact you to request additional information, correct a scheduling issue, change the appointment type, or reschedule an appointment.
7. Cancellation, Rescheduling, and Missed Appointments
You must provide at least 24 hours’ notice to cancel or reschedule an appointment.
Appointments canceled or rescheduled with less than 24 hours’ notice, as well as missed appointments, are subject to a cancellation fee equal to 50% of the scheduled appointment price.
The cancellation fee:
Is not payment toward a future appointment, service, procedure, product, or treatment;
May be charged to the payment method provided during booking when authorized;
May be required to be paid before another appointment is scheduled; and
Applies whether the appointment was scheduled online, by telephone, or through another method.
If you arrive late and there is not enough remaining time to perform the scheduled consultation or service safely and appropriately, the appointment may be shortened or rescheduled. The late-cancellation fee may apply.
SkanWell may evaluate documented emergencies or exceptional circumstances individually. Any waiver is discretionary and does not waive the policy for future appointments.
If SkanWell cancels an appointment, the cancellation fee will not apply.
8. Pricing, Payments, and Insurance
Prices displayed on the website or through the booking system are subject to change unless you have entered into a separate written agreement fixing the price.
The displayed price for a consultation or appointment includes only the service identified unless expressly stated otherwise.
Additional charges may apply for:
Laboratory testing;
Diagnostic services;
Medications;
Prescription products;
Medical supplies;
Skincare products;
Procedures;
Follow-up appointments;
Treatment packages; or
Other services.
Any estimate is not a guarantee of the final amount due.
Insurance coverage and reimbursement vary. Unless SkanWell expressly agrees otherwise in writing, you are responsible for:
Determining whether a service is covered;
Obtaining required referrals or authorizations;
Understanding deductibles, copayments, and exclusions; and
Paying amounts not covered or reimbursed.
A failure by an insurer to reimburse you does not eliminate your payment responsibility.
9. Electronic Communications
By providing a telephone number or email address, you authorize SkanWell and its authorized service providers to contact you regarding:
Appointment requests;
Confirmations and reminders;
Intake forms;
Scheduling changes;
Treatment instructions;
Follow-up needs;
Billing and payment matters;
Safety information; and
Other communications related to services you requested.
Communications may be made by telephone, voicemail, text message, email, secure portal, or mail.
Message and data rates may apply.
Standard email and text messaging may not be fully secure. Do not use unsecured communication methods to send detailed medical information unless specifically instructed.
Consent to receive promotional communications is optional and is not required to receive medical care.
10. Privacy
Use of the website is subject to the Website Privacy Policy.
Clinical information and protected health information may also be governed by the Notice of Privacy Practices of SkanWell Medical Health PLLC.
PatientNow and other independent third-party services may present additional privacy notices and terms.
11. Accuracy of Website Content
SkanWell attempts to provide useful and reasonably current information. However, medical knowledge, laws, product availability, pricing, provider availability, and service offerings may change.
We do not warrant that website content is:
Complete;
Current;
Error-free;
Applicable to every person;
Available in every location; or
Suitable for a particular purpose.
We may modify or remove content, services, prices, and features without notice.
12. Individual Results and Testimonials
Individual responses to medical, wellness, skin-health, weight-management, and aesthetic services vary.
SkanWell does not guarantee:
Eligibility for treatment;
A particular medical or aesthetic outcome;
Weight loss;
Symptom improvement;
Treatment duration;
Satisfaction;
Freedom from complications; or
Results similar to those experienced by another person.
Photographs, testimonials, reviews, case examples, and patient experiences are illustrative only and do not predict or guarantee your results.
When testimonials or photographs involve an actual patient, legally required authorization or consent will be obtained before publication.
13. Products and Third-Party Information
The website may discuss products, medications, devices, laboratories, pharmacies, manufacturers, or other third parties.
Such references do not necessarily constitute:

You represent that information you submit through the website is accurate and that you have the right to provide it.
Do not submit content that:
Violates another person’s privacy or intellectual-property rights;
Is unlawful, threatening, defamatory, obscene, or fraudulent;
Contains malicious code;
Impersonates another person;
Misrepresents your identity or authority; or
Interferes with website operations.
Health information submitted for treatment, payment, or healthcare operations will be handled under applicable privacy requirements and is not governed by any public-content license.
Feedback or general suggestions that do not contain protected health information may be used by SkanWell without compensation or obligation, unless otherwise agreed in writing.
16. Prohibited Uses
You may not use the website to:
Violate a law or regulation;
Attempt unauthorized access to an account, system, server, or network;
Introduce viruses, malware, or harmful code;
Interfere with website security or operation;

Harass SkanWell personnel or other users;

SkanWell does not control and is not responsible for a third party’s:

Availability;
Accuracy;
Security;
Privacy practices;
Products;
Services; or
Terms.
Your interaction with a third party is between you and that third party.
A link does not necessarily mean that SkanWell endorses the third party.
18. Website Availability
We do not guarantee uninterrupted access to the website, PatientNow scheduler, patient portal, or any other online feature.
Access may be interrupted by:
Maintenance;
Technical problems;
Cybersecurity events;
Vendor outages;
Internet failures;
Updates; or
Circumstances beyond our control.
Contact SkanWell by telephone when the scheduler is unavailable and you need non-urgent scheduling assistance.
19. Disclaimer of Warranties
To the maximum extent permitted by law, the website and its content are provided “as is” and “as available.”
SkanWell disclaims warranties of every kind, whether express, implied, or statutory, including implied warranties of merchantability, fitness for a particular purpose, title, and non-infringement.
Nothing in this section limits obligations that cannot lawfully be waived or duties arising from an established patient-provider relationship.
20. Limitation of Liability
To the maximum extent permitted by law, SkanWell and its owners, members, shareholders, managers, officers, directors, employees, contractors, providers, licensors, and service providers will not be liable for indirect, incidental, special, exemplary, consequential, or punitive damages arising from or relating to:
Use of or inability to use the website;
Reliance on general website content;
Website interruptions;
Third-party links or services;
Unauthorized access not caused by legally actionable misconduct by SkanWell; or
Loss of data, profits, business, or goodwill.
Nothing in these Terms excludes liability that cannot legally be excluded, including liability arising from professional medical services when applicable law prohibits limitation.
21. Indemnification
To the extent permitted by law, you agree to indemnify and hold harmless SkanWell and its affiliated entities, owners, officers, employees, contractors, and agents from claims, liabilities, damages, and reasonable expenses arising from:
Your unlawful use of the website;
Your material violation of these Terms;
Information or content you submit in violation of another person’s rights; or
Your fraudulent, abusive, or malicious conduct.
This provision does not require a patient to indemnify SkanWell for SkanWell’s professional negligence, unlawful conduct, or violation of an applicable legal duty.
22. Governing Law and Venue
These Terms are governed by the laws of the State of New York, without regard to conflict-of-law principles.
Unless applicable law requires otherwise, a legal action concerning the public website or these Terms must be brought in a state or federal court with jurisdiction in Onondaga County, New York.
This section does not alter any rights or procedures that apply to professional-liability, privacy, consumer-protection, or other claims under applicable law.
23. Changes to These Terms
We may revise these Terms when our services, technologies, or legal obligations change.
Revised Terms will be posted with a new effective date.
Your continued use of the website after revised Terms become effective constitutes acceptance of the revised Terms, except where affirmative consent is required by law.
24. Severability
If any provision of these Terms is held invalid or unenforceable, that provision will be enforced to the maximum extent legally permitted, and the remaining provisions will continue in effect.
25. No Waiver
A failure to enforce a provision of these Terms does not waive the right to enforce that provision later.

These Terms, together with the Website Privacy Policy and any additional terms presented for a particular website feature, constitute the agreement governing use of the public website.
Separate agreements, patient consents, financial policies, clinical forms, and PatientNow terms may govern appointments, treatment, payments, and patient services.
