Cancellation Policy
Appointments must be canceled or rescheduled at least 24 hours before the scheduled appointment time.
Cancellations made with less than 24 hours’ notice and missed appointments will be charged a fee equal to 50% of the scheduled appointment price.

Appointment Confirmation and Eligibility
Submitting an appointment request through the PatientNow scheduler does not guarantee eligibility for any particular treatment, medication, prescription, product, or procedure.
All medical services are subject to an individualized assessment by an appropriately licensed healthcare professional. Additional information, laboratory testing, an examination, medical clearance, or referral to another healthcare provider may be required before treatment can be recommended or provided.
SkanWell may contact you if additional information is needed or if the consultation type you selected does not appear to match your stated needs.
Cancellation, Rescheduling, and Missed Appointments
SkanWell requires at least 24 hours’ notice to cancel or reschedule an appointment.
Appointments canceled or rescheduled with less than 24 hours’ notice, as well as missed appointments, will be subject to a cancellation fee equal to 50% of the scheduled appointment price.
The cancellation fee is not payment toward a future appointment, treatment, product, or service.
An appointment may need to be rescheduled if you arrive late and there is not enough remaining time to provide the scheduled service safely and appropriately. When this occurs with less than 24 hours’ notice, the cancellation fee may apply.
SkanWell may consider documented emergencies or exceptional circumstances on an individual basis. Any waiver is provided at SkanWell’s discretion and does not waive the policy for future appointments.
Consultation Fees and Additional Costs
The price displayed during booking applies only to the consultation or appointment selected unless expressly stated otherwise.
Consultation fees generally do not include laboratory testing, diagnostic services, medications, prescriptions, skincare products, treatments, procedures, treatment packages, or future appointments. Applicable costs will be reviewed before additional services are provided whenever reasonably possible.
Patients are responsible for understanding their insurance benefits and for charges that are not covered or reimbursed by their insurance plan.
No Guarantee of Treatment or Results
Scheduling or completing a consultation does not guarantee that a particular service, medication, prescription, or procedure will be recommended.
Recommendations are based on your individual concerns, medical history, examination findings, laboratory results, treatment goals, potential risks, contraindications, and the clinical judgment of your healthcare provider.
Individual experiences and outcomes vary. SkanWell does not guarantee a particular result, degree of improvement, duration of benefit, or eligibility for treatment.
Medical Emergencies and Urgent Concerns
The website and online scheduling system are not monitored for emergencies or urgent medical concerns.
Do not use the scheduler to report a serious reaction, request emergency assistance, or communicate time-sensitive medical information.
If you believe you are experiencing a medical emergency, call 911 or go to the nearest emergency department.

Use the PatientNow system or another SkanWell-approved secure method when submitting health information or medical documents.

Telehealth Appointments
When telehealth consultations are available, you may be required to confirm your physical location at the beginning of the appointment.
You must be located in a state or jurisdiction where the treating provider is authorized to practice at the time services are provided.
Telehealth may not be appropriate for every concern. Your provider may require an in-person examination, laboratory testing, medical clearance, emergency evaluation, or assessment by another healthcare professional before making recommendations or providing treatment.
Technical issues or clinical circumstances may require rescheduling or converting an appointment to an in-person visit.
Clinical and Administrative Services
SkanWell is the brand under which affiliated clinical and administrative services are coordinated.
Medical evaluations, diagnoses, prescriptions, procedures, and other professional healthcare services are provided exclusively by appropriately licensed healthcare professionals through SkanWell Medical Health PLLC.
Scheduling, technology, marketing, administrative support, and other non-clinical services may be provided by SkanWell Management LLC.
SkanWell Management LLC does not practice medicine, prescribe medication, determine treatment eligibility, or control the independent professional judgment of licensed healthcare providers.

This Notice applies to SkanWell Medical Health PLLC, its healthcare professionals, workforce members, and locations operating under the SkanWell name.
SkanWell Management LLC may provide administrative, management, scheduling, technology, billing-support, and other non-clinical services to SkanWell Medical Health PLLC. When SkanWell Management LLC or another service provider creates, receives, maintains, or transmits protected health information on behalf of SkanWell Medical Health PLLC, it may do so only as permitted by applicable law and the agreements governing that relationship.
Your Rights
When it comes to your health information, you have certain rights. This section explains those rights and some of our responsibilities.
Obtain an Electronic or Paper Copy of Your Medical Record
You may ask to inspect or receive an electronic or paper copy of your medical record and other health information maintained about you.
We will generally provide a copy or summary within 30 days after receiving your request. We may charge a reasonable, cost-based fee as permitted by law.
Certain information may be withheld or access may be limited when permitted or required by law. If we deny access, we will explain the reason and tell you whether the decision may be reviewed.
Ask Us to Correct Your Medical Record
You may ask us to amend health information that you believe is incorrect or incomplete.
We may deny your request in certain circumstances, such as when we did not create the information or determine that the existing record is accurate and complete. If we deny your request, we will explain the reason in writing, generally within 60 days.
Request Confidential Communications
You may ask us to contact you in a particular way or at a different telephone number, email address, or mailing address.
We will accommodate reasonable requests. You do not need to explain why you are making the request.
Ask Us to Limit What We Use or Share
You may ask us not to use or disclose certain health information for treatment, payment, or healthcare operations.
We are generally not required to agree to such a request. If we agree, we will comply with the restriction except when the information is needed to provide emergency treatment or another disclosure is required by law.
If you pay for a healthcare item or service entirely out of pocket, you may ask us not to disclose information about that item or service to your health plan for payment or healthcare operations. We will honor that request unless disclosure is required by law.
Receive an Accounting of Disclosures
You may ask for a list, known as an accounting, of certain disclosures of your health information made during the six years before the date of your request.
The accounting will not include every disclosure. For example, it generally will not include disclosures made for treatment, payment, healthcare operations, disclosures you authorized, or certain other disclosures excluded by law.
We will provide one accounting in any 12-month period without charge. We may charge a reasonable, cost-based fee for additional accountings requested within the same 12-month period.

Choose Someone to Act for You

People Involved in Your Care

We will obtain your written authorization before using or disclosing psychotherapy notes in most circumstances in which such notes exist and authorization is required by law.
Fundraising
SkanWell does not currently use protected health information for fundraising communications.
If that practice changes, you will have the right to opt out of future fundraising communications.
How We May Use and Disclose Your Health Information
Treatment
We may use and disclose your health information to provide, coordinate, and manage your care.
For example, we may disclose information to:
Physicians, physician assistants, nurses, laboratories, pharmacies, or other professionals participating in your care;
A specialist or another provider to whom you are referred;
A pharmacy processing a prescription;
A laboratory performing testing ordered for you; or
A healthcare professional from whom we request consultation.
Payment
We may use and disclose health information to bill for and collect payment for services provided to you.

When you interact with the embedded scheduler or PatientNow services:

PatientNow may process information as a service provider to SkanWell;
Additional PatientNow terms, notices, or consents may be presented;
Information may become part of your patient or prospective-patient record; and

Information may be transmitted directly to PatientNow;
Information may become part of your prospective-patient or medical record; and
PatientNow’s terms and privacy practices may apply to its independent services.
You are responsible for providing accurate, current, and complete booking information.
An appointment is not guaranteed until you receive confirmation.
SkanWell may contact you to request additional information, correct a scheduling issue, change the appointment type, or reschedule an appointment.
7. Cancellation, Rescheduling, and Missed Appointments
You must provide at least 24 hours’ notice to cancel or reschedule an appointment.
Appointments canceled or rescheduled with less than 24 hours’ notice, as well as missed appointments, are subject to a cancellation fee equal to 50% of the scheduled appointment price.
The cancellation fee:
Is not payment toward a future appointment, service, procedure, product, or treatment;
May be charged to the payment method provided during booking when authorized;
May be required to be paid before another appointment is scheduled; and
Applies whether the appointment was scheduled online, by telephone, or through another method.
If you arrive late and there is not enough remaining time to perform the scheduled consultation or service safely and appropriately, the appointment may be shortened or rescheduled. The late-cancellation fee may apply.
SkanWell may evaluate documented emergencies or exceptional circumstances individually. Any waiver is discretionary and does not waive the policy for future appointments.
If SkanWell cancels an appointment, the cancellation fee will not apply.
8. Pricing, Payments, and Insurance
Prices displayed on the website or through the booking system are subject to change unless you have entered into a separate written agreement fixing the price.
The displayed price for a consultation or appointment includes only the service identified unless expressly stated otherwise.
Additional charges may apply for:
